This is where you will be able to find all the official forms and paperwork that you might need in order to obtain your Colorado MMJ License or to change your caregiver, report a lost MMJ card, or request for Patient information.

Please note that most of these forms are new as of March 1st, 2010!

The first and primary form that you will need to start it all of with is the
This is a three-page PDF application form. The first page will provide the State of Colorado's Health Department the information about both you and your physician. If you decide to have an official caregiver at this time, you will also provide that information as well on this form. (Please realize that your choice of caregiver is a very important one! You never have to have a caregiver and never assign a caregiver to your MMJ license because of a cheap doctor’s deal! You the patient own all your plants, make a deal representative of YOUR needs. Ask us about this situation and also ask what we might be able to provide to you as the Ganja Gourmet being your primary caregiver.) This application has been revised as of March 01, 2010.

The second form listed here is the CDPHE
If you should have a change of address or if you decide to change your caregiver, this is the official form to use. (Never be afraid to end your patient/caregiver relationship and get a better caregiver. It is strictly the patient’s choice and the patient is not legally bound to ever keep a caregiver they do not want!) This application form has been revised as of March 01, 2010.

The third form that is available for your use is the
Please use this form if you should ever lose your license. Should your card get too messed up to use anymore, this is also the form to use to get the replacement license.

The fourth form that is available is the application for Patients that are under the age of 18. This is a new application form dated March 01, 2010.
